Required Documents (You will be asked to upload them):

  • Copy of Driver's License 
  • Proof of Income (Include one of the following: Recent Paycheck Stub, SSI/SSDI Benefits, Federal or State Assistance, Unemployment Benefit Letter.)

Paws for a Purpose Application

Have you applied to any of the following? If so, list the amount approved.

If approved, we would like you to provide a picture of your pet.  We use pictures and stories to promote good will. 


Before submitting an application, notify the veterinary office that the applicant is seeking financial assistance and give them permsision to release information to Paws for a Purpose. 


We cannot help if any of the following apply:

  • Treament has been completed
  • Ongoing conditionds or treatments (diabetes or chemotherapy)
  • Routine care (spay/neuter, vaccinations or annual exams)

We cannot provide assistance for all medical conditions.
